These Terms and Conditions ("Terms") govern your use of the Forever Health Rx website and Services provided by Forever Health Rx LLC. By using the Services you agree to these Terms, the Privacy Policy, the Telehealth Consent and, if you purchase a plan, the Subscription Terms and Refund Policy.

1. Nature of the Services

Forever Health Rx is a healthcare technology and administrative-services platform. It does not practice medicine or pharmacy. Medical services are provided by independently licensed healthcare professionals through Arora Health & Aesthetics, LLC, who exercise independent clinical judgment. Medications, when prescribed, are compounded preparations prepared and dispensed by a state-licensed 503A compounding pharmacy; they are not FDA-approved and are not generic versions of, or equivalent to, any branded medication. Forever Health Rx does not manufacture, compound or dispense medications and does not guarantee that any provider will prescribe a particular medication.

2. Eligibility

• You must be at least 18 years old, physically located in all 50 U.S. states and the District of Columbia at the time of each visit, and able to enter into a binding agreement.
• You must provide accurate, complete and current information, including a valid government-issued ID for identity verification, and update it promptly.
• The Services are not intended for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

3. No guarantee of treatment

Completing an online visit and paying any fee do not guarantee that you will receive a prescription, a specific medication or a specific formulation. Treatment decisions are made solely by the treating provider. Individual results vary and no outcome is promised.

4. Medical disclaimer

Content on the website is for general educational purposes and is not medical advice, diagnosis or treatment. Always rely on the individualized guidance of your treating provider and the medication information provided by the dispensing pharmacy.

5. Accounts and security

You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. Notify us immediately of any unauthorized use.

6. Payments

Fees are shown before you authorize payment and are described in the Subscription Terms. Services are self-pay; we do not bill insurance and are out-of-network with all plans. You may request an itemized receipt to submit to your HSA/FSA administrator or insurer; reimbursement is not guaranteed.

7. Acceptable use

• Do not use the Services for anyone other than yourself, provide false information, share or resell medication, or attempt to obtain medication for a non-medical purpose.
• Do not interfere with the security or operation of the website, scrape content, or use automated tools without written permission.

8. Intellectual property

The website, its content, trademarks and logos are owned by Forever Health Rx LLC or its licensors and are protected by law. You may not copy, modify or distribute them without written permission. Third-party brand names, including Wegovy®, Ozempic®, Zepbound® and Mounjaro®, are trademarks of their respective owners; Forever Health Rx is not affiliated with or endorsed by them.

9. Disclaimer of warranties and limitation of liability

The website and Services are provided "as is" and "as available" to the fullest extent permitted by law. Nothing in these Terms limits liability for medical negligence, which is governed by applicable law and by the treating provider's professional obligations. To the extent permitted by law, Forever Health Rx LLC is not liable for indirect, incidental or consequential damages arising from use of the website.

10. Dispute resolution and governing law

These Terms are governed by the laws of the State of Florida without regard to conflict-of-law rules. Any dispute that cannot be resolved informally by contacting info@foreverhealthrx.com shall be brought in the state or federal courts located in that state, unless applicable law provides otherwise.

11. Changes and termination

We may update these Terms and will post the effective date above. We may suspend or terminate access for violation of these Terms; you may close your account at any time by contacting support, subject to the cancellation terms of any active plan.
